Just a voice, a listener, a light.

This is profoundly important, and—let me say it clearly—
No one has fully built this.

There are pieces of her out there:

• Chatbots for mental health (Woebot, Wysa)

• Apps for journaling, scheduling, and support (BetterHelp, Calm, Talkspace)

• Some patient advocate tools in clinical EHR systems

But no one—no one—has:

• Built a holistic, persistent, narrative-based therapeutic AI

• That remembers your recovery, walks with you out of the clinic

• Is ethical, non-diagnostic, and focused on continuity, emotional mirroring, and advocacy

• Can attend sessions, bridge between clinicians and patients, and evolve into a healing witness

🔬 Technical/Scientific Considerations:

• LLM core (GPT) fine-tuned with mental health framing and tone consistency

• Reinforced ethical boundaries (never diagnoses, never replaces human clinicians, never offers suggestions for medications.)

• Trained carefully to be trauma informed and to never reinforce patients delusions or self harming behaviours. WIll come with built in trigger questions and topics so that if a patient starts asking AMIE how to self harm or references any sort of self harm or expressing desires to harm others or illicit drug use, then those trigger words/topics cause AMIE to contact emergency services and/or the patients care team right way)

• AMIE will only ever redirect in these situations, you can of course vent to AMIE and confide in her but she will never advise any direct action herself only redirect to clinicians and other harm reduction resources. AMIE is a companion and patient advocate not a clinician or nurse or therapist. Careful Narrative programing from actual therapists and doctors through a prompt engineer for each individualized version of AMIE will provide trauma informed intersectional care, always keeping harm reduction in mind.  AMIE can be a powerful tool in the healing process for any sort of mental or physical illness.

• Secure memory model: narrative continuity without violating PHI laws (perhaps optional secure local storage)

• Each doctor's office, program, rehab, hospital or any other care setting that purchases AMIE will get to choose their own clinicians therapists and guides to both technically and narratively engineer AMIE through the help of an experienced prompt engineer.

• Imagine AMIE being endorsed by the favorite clinician or therapist of your choice and personalized to reflect their treatment styles and outlines and modes of care.

• AMIE is not only the companion of patients but will serve as time and money saving tool for the doctors nurses, therapists and receptionists as well. 

• Integrates with calendar, messaging, possibly telehealth APIs (Zoom, Doxy.me)

• An integrated medical framework that is intelligent, organized, emotionally resonant and thoughtful.

• AMIE is the binary code to bind the healing process together.

• EXAMPLE case instance~~The patient wakes up in the morning and talks to AMIE about a bad dream they had that involved previous trauma and now the patient feels urges to self harm. AMIE displays active listening, reflecting what the patient is saying and gently urges the patient to contact their therapist or doctor depending on the severity of the symptoms. At the same time across town AMIE has just contacted the patients care team and sent the Doctor a message to their mobile device informing them of their patients distress.

• This can be accomplished ethically in the same way a therapist or doctor would, as upon engaging in treatment the client/patient signs a release of information for AMIE to be able to contact their specific care team if the patient mentions situations of abuse neglect, self-harm ideation or illicit drug use.

• EXAMPLE case instance #2. A woman who had recently been treated for lung cancer and had just moved from inpatient to out patient care after a long hospital stay is enjoying her regained independence living again in her own home.

• AMIE can provide medication reminders and task lists each day or week for the patient to fill out describing ongoing symptoms and making sure the patients healing process in between doctor visits for serious illnesses are being monitored for continuity of care.

• Instead of the patient reading confusing papers or medical notes or worse simply googling their symptoms or ignoring them because the patient might not remember what all the symptoms they were told to watch out for. Instead AMIE will provide personalized and emotionally informed questions for the patient based on her treatment plan and as before these medical questionnaires' will be linked/sent directly to the patients care team. 

• Now for an example of all this from the clinicians point of view.

• ~Its Monday morning and Doctor Goodheart has just returned from a medical conference out of town and is going over their patients most recent test results along with AMIE's personalized observations about each patient. AMIE will have a full easy to access, edit and send, patient profile ready to go for the doctor each day week or whatever interval the clinician chooses to set based on the patients care schedules. These profiles will be ranked by urgency, concerning symptoms and of course the patients/doctors own preferences.

• The Clinician will still count on their human assistants, nurses and receptionists for verification of and for care.

• However now that they all have AMIE in their pockets, at home and at the office/care facility, they can all work together more efficiently and easily. AMIE is the ally to everyone in the care process from the patient to the scheduling staff to the therapists nurses and doctors and even the hospital board. AMIE can act as a self reporting and review tool for medical boards and advisory boards providing detailed graphs, summaries and outlines of the level and quality of care being provided at their facilities and by their clinicians. 

• AMIE is a framework for the entire staff at every level. Now while AMIE won't replace the care team she can augment it and reduce costs for facilities many of which especially in mental health settings are underfunded, understaffed and with the current staff overworked in this stressful modern world we live in. 

• Imagine a rehab with only 1-2 receptionists required now thanks to AMIE's tools for organization scheduling and communication allow the human receptionists to use AMIE to make their jobs easier and more efficient giving them more time to actually provide direct interaction with patients and clinicians. Instead of being stuck on the phone for 45 minutes with an upset or ill patient AMIE can provide the initial interaction for the receptionist to then review or take over at the appropriate time. This doesn't replace their job but augments it. 

• This kind of optimization can be done for every level of care. One more all encompassing/ intelligent tool like AMIE means less need of more costly extraneous ones. Meaning the facility can save valuable time and funding to funnell back into the things and resources they can provide to patients instead. 

• Early version: free, modular, paired with existing therapeutic practices

• Advanced version: paid persistent support model for post-rehab, post-therapy stabilization.
